If a member of staff reports an accident, incident or dangerous occurrence, they are required to:

  • Complete the correct form (IR2 for staff accidents and/or H&S related incidents)
  • Ensure the form is completed in full and within 24 hours of the accident or incident
  • Ensure details recorded are accurate and that there is sufficient detail on the form and the writing is legible
  • Attend the Accident and Emergency, or Occupational Health department, when advised to by their manager
  • Make sure the form is given to their manager so that they can investigate appropriately (unless there are exceptional circumstances, for example, if you are being bullied by your line manager, in which case you would contact the H&S Advisor)
  • Include the full names and contact numbers of any witnesses on the form
