Within the NHS there are different forms to be completed for different types of incidents being reported. The organisational Incident Reporting Policy will cover incident reporting arrangements in detail, and will include:

  • Reporting incidents to the HSEaccident report
  • Management and staff responsibilities
  • The incident investigation process

Non-clinical incidents include accidents, incidents and near-misses which relate to a failure or breach of H&S, regardless of who is injured or involved.

The non-clinical form records all non-clinical accidents and incidents to patients, staff and others as well as any case of suspected or known work related ill-health.

Note that clinical incident reporting and clinical management is normally looked after by Risk Management.
